Service Subject
- Referral Service: Individuals between age from 7 to 64, with registered residence or actualdwellingin Xinan District, Tainan City, thephysical and mental who are disabled equipped with theproof certification.
- Individual CaseManagement Service: Individuals between age from 18 to 64, with registered residence or actualdwelling in Xinan District, Tainan City, the physical and mental who are disabled equipped with the proof certification or suspected to be physical and mental who are disabled.
- Aging Family Supporting Service of Mental who is Disabled: Individuals above age 35 and dwelling with Mental who is Disabledbut as the primary caring person above age 60 living in the community.
Content of Service
- Report Acceptance: Accept reports from all sectors or ask for servicing of individual himself which evaluated by social workers.
- Consultation Service: According to the needs of the service object requesting and the ability of resource utilization, provide consultation of short-term supporting service and welfare service
- Career Referral: Provide assistance to whom have single service demanding in school study, employment, caring, and medical treatment for the purpose ofgetting appropriate services during different stages of their lives toperform career referral.
- IndividualCase Management: Provide services to whom having multiple demands thensatisfying their needsand improving their independent abilities through linking to the utilization of various resources to customize service plan establishment and execution, thereby solving the current problem.
- Aging Family Supporting Service of Mental who is Disabled:Utilize theservice modelof individual case management, link to the assistance of resources, provide supporting services of experts visiting familiesdepending upon the family needs, resource referral,caring and visiting of familyand telephone caring service.
Purpose of Service
- Assist constructing community resources of the physical and mental who are disabled and their families to improve living supporting system soundness.
- Provide complete and continuous service to satisfy the diverse needs of the physical and mental who are disabled and their families.
- Protect the rights and benefits ofthe physical and mental who are disabled, implement various welfare services to them.
